COUNTY SUB CERTIFICATE/CRIMINAL HISTORY

1)  GO TO www.bioapplicant.com/nj and make an appointment. The cost is the responsibility of the person, not the school. ($70.25 is the fee payable online)… The form you need to make your appointment is available at SICPS.

2)  Return the APPLICANT AUTH FORM with PCN number and notary with a $7.00 money order payable to the State of NJ. You must have a money order. We will sent out the transmittal form/employee roster for you.

3)  Once Criminal History is cleared through the State of NJ, we mail out your COUNTY SUBS APPLICATION with $125.00 money order and an official copy of your college transcripts (60 credits)- which you must formally request from your college/university.
